What is Medication Management?
Medication management is the process of administering, dispensing, and working with doctors to ensure proper dosage and management of the side effects of prescribed medications. Our team of licensed medical professionals can administer medications as well as answer your questions about medications.
How IADC can help with medication management:
- Medication Administration
- Monitoring and Adherence
- Monitoring Side Effects
- Interactions and Compatibility
- Education and Communication
- Storage and Disposal
- Regular Check-ins
Each of these tasks is an important aspect of the management process because it helps ensure that your loved ones receive the maximum benefits from their treatments while minimizing the risks associated with incorrect usage. Healthcare providers, pharmacists, caregivers, and patients themselves all play vital roles in effective medication management.
Why Our Medication Management Services Are Helpful
Even the most organized individual can get confused sometimes about medications, especially when you’re balancing other types of care for your loved one and yourself. IADC employs nurses and licensed Medical Assistants to ensure that medications are administered properly and on time.
You might benefit from medication management services if:
- You struggle to understand the effects of medications
- You are not seeing the desired outcome of medications
- It’s hard to keep track of how medications interact with each other
- You struggle to keep track of administration times
- Your loved one suffers from visual or cognitive impairments and is at risk of skipping a dose or administering the wrong dose.
Many caregivers receive a great deal of relief from our medication management services because it removes confusion about a loved one’s medication schedule as well as ensures proper administration.
Health Assessments at IADC
One of the first things we do when enrolling a new guest at IADC is a comprehensive health assessment. This assessment allows us to understand all the unique care needs of each guest. It includes a review of each guest’s medical history and medication list. This allows us to develop an individualized plan of care to be followed while the guest attends the center and/or participates in any services offered by IADC.
Medication Management Services in Centers or At Home
As part of our medical services, we make sure that our guests don’t miss a dosage of their important medication while they’re in our care. There are multiple ways we can ensure that your loved one gets the medications they need.
In-Center Medication Management Services
Each IADC Center has medical professionals on staff to support and supervise proper medication management and administration of our guests. Our center nurse can administer medications for guests during the day while they are enjoying their time at the center. Over-the-counter medications or emergency medications can also be administered by the center nurse as needed. Our team securely stores medical records and medications and ensures that they are up to date. Our team will make sure that your medications are refilled in a timely manner as well.
At-Home Medication Management Services
Through Independent Adult Day Centers’ continuum of care services, medications can also be managed by our Healthcare Coordination or Home Health Services. We can help make medication management, oversight, and prescription refills easier for guests and caregivers in the comfort of their homes. Medications can include IVs and injections as well.
Guests who are enrolled in IADC’s Healthcare Coordination services, also benefit from a medical professional who is available to attend doctor’s appointments and follow up on your behalf. This way information about medication updates and doctor’s orders can always be clearly conveyed.
